Transitions 5 to 11

A well-planned transition ensures that all information is communicated clearly to the new setting to meet the child where they are in order to provide positive outcomes.

Skills for School

The Educational Psychology Service has produced a checklist for parents/carers about the skills needed to prepare a child for school, download: Skills for School Checklist (OpenDocument text format, 10 KB)(opens new window)

Transition into a new setting

Planning and preparation for transition may involve a range of different elements, though these are not necessarily separate activities. They are likely to include:

  • establishing relationships
  • sharing information
  • visits
  • creating continuities
  • preparation in the setting
  • training and support
  • settling in and follow up
