Transitions 11 to 16

Wondering what the next big step in your education or life is going to look like?

Parent carers informed the development of a Transitions Pathway Map. The map begins at Year 9 and carries through to early adulthood, and takes you through the information you need to help with planning at various stages in your young person's progression towards adulthood.

The following drop-down menus break the information on the map into manageable chunks and is arranged in school years from year nine. 

Additional support and information is available to help children and young people when moving through different phases of education.  All maintained nurseries, schools and academies are required to  publish a SEN information report which provides information about the support they offer for children and young people with special educational needs and/or disabilities within their care.
