Stepping forward to adult services

Most health services change at 18 years of age to those that support adults. Many of your services like GPs (NHS) and Urgent and Emergency Care will not change as they are all age support.

Stepping forward from children's continuing healthcare to adult healthcare is a significant step for young people and their families. This process, often referred to as 'healthcare transition', involves moving from a paediatric to an adult model of care.
