Additional support and information is available to help children and young people when moving through different phases of education. All maintained nurseries, schools and academies are required to publish a SEN information report which provides information about the support they offer for children and young people with special educational needs and/or disabilities within their care.

Early Years Settings and Schools can access support and advice around early years transitions for individual children from their Early Years Inclusion Advisor.

Transition materials and training for support workers
The Early Years Service includes Early Years Advisory Teachers, Quality Assurance Officers and Early Years Inclusion Officers meaning they can support you on all aspects of early years provision. The team also host a resource section which features, transition journals, transfer information and how to conduct transition meetings.
