Learn about the Outreach and Enablement Service (OES) and the Intensive and Enablement Service (IES).
Our OES team is trained to help you achieve your goals, access your community, learn new skills and to gain confidence. Our IES team is trained to help you with problem solving skills, wellbeing and building your independence.
Accessing enablement services
Individuals 18 and over can be referred to the Enablement Teams.
IES provide time limited support of up to 6 weeks. OES provide time limited support of up to 12 weeks. This is reviewed on a case-by-case basis. The allocated worker must remain involved throughout our involvement.
The teams operate from 9am to 5pm but are flexible in its approach to meet the requirements of the people we are supporting.
Individuals referred to the service who are at risk of placement breakdown, provider breakdown, admission/readmission into hospital or require support upon discharge from hospital will be prioritised according to service capacity.
The Enablement Service is not a registered service and therefore does not provide personal care or medicine administration. The service is person-centred and varies according to the needs of the people referred.
If an individual has been assessed as not having eligible needs in accordance with the Care Act 2014, they can be referred to the Enablement services, if outcomes have been identified in which the individual is able to be supported to work towards and/or to prevent further Social Care intervention.
OES focuses on maximising independence for people aged 18+ with learning disabilities and/or Autism.
The aim of service is to build upon an individual's strengths, knowledge and understanding of life skills to build confidence, enable and empower individuals to promote independence within their own lives.
Working with the individual and allocated worker the service will provide time limited support of up to 12 weeks in which individuals accessing the service are supported to achieve outcomes on their pathway towards independent living.
The support that is offered is flexible depending on each individual need and their outcomes. In addition, The team will work closely alongside the Family and Learning team to provide 'SMART' courses to individuals.
Qualified tutors will deliver learning on specific areas including health and wellbeing, finances, communication, social skills and food/nutrition.
Once an individual has been referred to the service and is supported to access the course it is then the team's role to embed the learning throughout the support.
The Intensive and Enablement Service (IES) focuses on supporting individuals aged 18+ with Learning Disabilities and/or Autism with Mental Health.
Individuals referred to the service may be at risk of crisis, in a crisis, risk of hospitalisation, risk of placement breakdown or may require a period of enablement to build on their independent living and problem-solving skills.
The service also offers support to individuals upon discharge from hospital.
Focusing on individuals' strengths and resilience the support will be provided in their homes, or in the community to develop their daily living skills to improve their quality of life. Therefore, the outcomes we seek are supporting positive wellbeing, feeling good and staying well.
The Team work with individuals for an intensive time period of up to 6 weeks in which we carry out a number of visits to support them to achieve agreed outcomes.
The support that is offered is flexible depending on each individual need and their outcomes.
