16+ SEND provision survey

To help us gather information we have created three questionnaires. The first two are aimed at our young people, pre and post 16 years of age, to capture the experiences of those at the point of navigating towards their post-16 provision, and those who have undertaken the journey already, to tell us their experience of it. The third questionnaire is for providers, asking them what they offer to our post-16 young people with SEND.

This is for 14-16 year olds with SEND currently navigating the process of moving to their post-16 provision.

This for Post-16 young people with SEND to help us understand what they experienced when they moved into post-16 provision.

Questionnaire 3: Provider

This is for our providers. We would like to know more about the provision that is currently offered to Post-16 young people with SEND. This will allow us to map the provision for young people and their families.

The questionnaires close on Monday 3 July, please submit them before this date. It will help us to further understand the current SEND picture in Wiltshire, and where we can improve on our Post-16 SEND offer.

If you work with young people please share the questionnaires with them and, where necessary, help them to complete and return them.
